Chronic care management is billed once per calendar month. However, only one provider can bill for the rendered services in a calendar month.
High-risk Medicare beneficiaries whom multiple chronic conditions has put under the risk of death, functional or physiological decline.
Verbal consent must be obtained once prior to initiating CCM services.
The billing provider may bill either complex or non-complex CCM in a calendar month.
